To obtain your medical records, contact our Medical Records department at 401-348-3262 or outside RI, 1-800-933-5960, extension 3262. You will be asked to complete and sign a release form.

To obtain copies of your diagnostic image reports and/or discs containing your images (e.g., X-ray, MRI, etc.), contact Radiology at 401-348-3293 or outside RI, 1-800-933-5960, extension 4105.  Images can be picked up at the Radiology office.
