Numbers based on 2013 Federal Health and Human Services Poverty Guidelines. Guidelines change periodically so consult our financial services counselors for most current numbers.
How to Apply for Charity Care
To qualify for charity care, you will need to fill out the form: Application for Hospital Financial Assistance (for downloadable form click here). You will be asked to provide our Financial Services Department with copies of these documents, if applicable:

When we have received 60% of the total amount due, the Hospital will adjust the remaining balance to our Financial Assistance Program.
Discounted Care: If you do not qualify for full charity care, you may qualify for discounted care up to 40%. Speak with one of our financial counselors to find out if you qualify.
We offer payment plans for patients with balances remaining after insurance and for patients who do not qualify for charity care. Contact our Financial Services Department.
