Anti-Coagulation Clinic at The Westerly Hospital

 

The Anti-Coagulation Clinic at The Westerly Hospital provides the following services:

 

  • Regular consultation with clinical pharmacists trained in
    anticoagulation
  • Extensive warfarin education including written materials provided to each patient and education reinforcement at clinic visits
  • A comprehensive initial interview
  • Immediate test results and dosage adjustment
  • Fingerstick point of care INR readings rather than venipuncture
  • Wafarin prescriptions called into the patient’s pharmacy
