Management Services at The Westerly
Hospital
Through
the Care Management Department, The Westerly Hospital has a social worker
available for any personal issues of concern to you. The social worker provides
professional, compassionate services to help patients and their families
find solutions and ways to cope with many of life’s difficulties. These difficulties may include: coping
with your illness, family relationships, bereavement issues, advance
directives, financial concerns, psychiatric issues, alcohol/drug abuse
and any concerns you may have about abuse or neglect.
In
addition, the Care Management department provides Case Management and
Discharge Planning for hospital patients.
